VITALITAS DENVER CANCELLATION POLICY

Please review the following information about the Vitalitas Denver cancellation policy. Acknowledge your review, understanding and acceptance of this policy by checking the box at the bottom of this page.

We have implemented a cancellation policy to reflect the customized and time consuming nature of our infusion center practice.

It is difficult for us to fill last minute cancellations, and we have an increasing amount of patients who want to be seen for this treatment with limited availability. We thank you for understanding this new policy.

One hour infusions will need to be cancelled at least 72 hours in advance or there will be a $325.00 cancellation charge. We will collect payment prior to your first scheduled treatment that will be applied to the first treatment cost. Any subsequent cancellations with less than 72 hours notice will incur a $325.00 charge to your credit card.

Four hour infusions will need to be cancelled at least 1 week in advance or there will be a $500.00 cancellation charge. We will collect payment prior to your first scheduled treatment that will be applied to the first treatment cost. Any subsequent cancellations with less than 1 weeks notice will incur a $500.00 charge to your credit card.

In order to respect the time of all of our patients, if you are more than 15 minutes late to your appointment, we may be unable to treat you and you will need to reschedule, incurring a $325.00 cancellation fee.
