Vitalitas Denver, PC is committed to protecting the privacy of your health information. We are required by federal and Colorado state law to maintain the confidentiality of your Protected Health Information (PHI) and to provide you with this Notice of our legal duties and privacy practices.
How We May Use and Disclose Your Health Information Without Your Authorization
Treatment: We may share your health information to provide, manage, and coordinate your care, including sharing with medical providers within Vitalitas Denver as well as outside providers involved in your treatment.
Payment: We may share your information as necessary to collect payment for products and services rendered.
Healthcare Operations: We may use and share your information for healthcare operations including quality assessment and improvement, development of clinical guidelines, management and care coordination, review of provider competence, medical review, auditing, fraud detection, and business management.
As Required by Law: We may disclose your information when required by federal, state, or local law, including for public health activities, law enforcement purposes, or in response to a court order or subpoena.
Substance Use Disorder Records: Records relating to substance use disorder treatment are subject to additional federal protections under 42 CFR Part 2 and may not be disclosed without your written consent except as specifically permitted by law.
Uses and Disclosures Requiring Your Authorization
For any use or disclosure not described above, we will ask for your written authorization. You may revoke that authorization in writing at any time, except where we have already acted in reliance on it. Authorization forms are available upon request.

You have the right to:
Access and copy your health records (with limited exceptions)
Request amendments to information you believe is incorrect or incomplete
Request restrictions on how your PHI is used or disclosed
Request confidential communications (e.g., contact you only at a specific phone number or address)
Receive an accounting of disclosures we have made of your PHI
Receive a paper copy of this Notice at any time, even if you have agreed to receive it electronically

Our Duties
Vitalitas Denver, PC is required by law to maintain the privacy of your PHI, to provide you with this Notice, and to follow the terms of the Notice currently in effect. We reserve the right to change our privacy practices and the terms of this Notice. If we make a material change, we will update this Notice and post it on our website.
How to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized for filing a complaint.
