PAYMENT & CANCELLATION POLICY — VITALITAS DENVER, PC

I. Payment

By scheduling services with Vitalitas Denver, PC, you understand, acknowledge, and agree that you are fully responsible for payment of services and that payment must be made in full at or before the time of service. Current pricing is as follows:

  • 40-minute infusion: $375

  • 2-hour infusion: $700

  • 3-hour infusion: $975

  • 4-hour infusion: $1,350

Treatment and services provided by Vitalitas Denver may not be covered by your insurance. Vitalitas Denver and its physicians are non-participating providers and do not contract with any insurance plan. Vitalitas Denver will provide documentation to assist you in submitting a claim to your insurance carrier but will not submit claims to insurers on your behalf and is not required to speak with insurance companies on your behalf. We are also unable to complete paperwork for insurance companies.

You have the right to explore with Medicare, Medicaid, or other insurance companies whether outpatient ketamine intravenous infusion is a covered benefit under your plan and to find another practice that may contract with those programs.

If you have Medicaid/Health First Colorado, please be aware that it does not cover ketamine intravenous infusions given by a doctor in the outpatient setting for chronic pain or mental health. By receiving services at Vitalitas Denver, PC, you are entering into a private agreement to pay for your infusions. This agreement covers all future infusions as well.

If your insurance changes to Medicare or Medicaid, it is your responsibility to notify Vitalitas Denver, PC in writing before your next scheduled appointment so that proper paperwork can be completed. Vitalitas Denver, PC has opted out of Medicare. Any treatment with Vitalitas Denver, PC is a private contract and Medicare will not be billed by either party.

If you choose to submit any bill or itemized receipt to an insurance carrier for reimbursement, Vitalitas Denver, PC is exempt from any dispute regarding reimbursement.

II. Cancellation

To fairly and effectively serve all patients, the following cancellation policy applies:

40-minute infusions must be cancelled at least 72 business hours in advance to receive a full refund. Cancellations made less than 72 business hours in advance will incur a $150 charge. Business hours are Monday through Friday.

Two to four hour infusions must be cancelled at least two weeks in advance to receive a full refund. Cancellations made less than two weeks in advance will incur a $500 charge.

Appointments with Shantel Gallegos, PNP must be cancelled at least 24 hours in advance. Appointments not cancelled within that window will incur a $50 missed visit fee.

All charges are payable by funds prepaid to your account or charged to your credit card on file.
